I used to see framers using 2 pieces of 2x material with 1/2" plywood in between. I suppose it might be neat to replace the ply with a rigid foam?
Sometimes the builder forms in joist material, uses the forms for framing after.
In my area, we (the industry) use a 92-1/4" precut rather than CA's default of 92-5/8", so a 4x12 yields an 82-1/2" RO. Thus, all our windows are higher than our doors and everyone lives with it. Since I do additions and remodels, I just match the existing but -if- I were building new, I might consider an upside down Tee of 2x12 over 2x4 or 2x6 (per the wall) where possible.Or, an I-joist with foam either side. Or a pair of 1-1/4" rim boards with an inch of foam. So many choices,....Matt Kennedy
