I think you will find that there never is a + sign next to it in the used column in any anno set.
Just turn on the display column in the 1/4" anno set.

Once in a while, I find a layer that has different names in different layer sets. I would create a copy of your layer set that your ceiling break lines won't display, & then start turning layers on/off until you figure out what the layer name is in that layer set. You will get a "+" sign for that layer if you have added something to that layer yourself. Also, if a selection stays selected when changing layer sets, you could draw a line on the Ceiling Break Lines layer, select it & then change layer sets & see the layer name there. Once you find the problem, get into Layer Display Options(LDO)/ Modify All Layer Sets & correct the name.
Annotation sets have nothing to do w/ layer display issues, other than controlling what layer set displays when a particular anno set is activated. It's all done in the LDO. Why would you go to Anno Sets 1st?Thanks, Jim

Ed thanks for the reply, I didn't go to layer sets first I went to display options and when that was checked and after doing other work on different annotation sets I started checking both when that set was on I check the display options to make sure it was on in that annotation set also so that it was not turned off for some reason, so when I went to the different annotation sets I always check my display options to see, I will try your other ideas in a little while
Thanks

Ray,
A layer is not On/Off in an Anno Set. It's On/Off in a Layer Set.
Forget Anno Sets for this issue; they're irrelevant.
& all changes can be done thru the LDO; shouldn't have to open/close it multiple times.
You can send me the plan & I'll have it fixed in minutes, if what I suspect is indeed the problem; glad to do it.
Either way, you'll know soon.

Ray,
All fixed. Exactly what I thought it was.
It was the Walls, Beams layer.
Your "Ceiling Break Lines" layer appears to be a user added layer.
I deleted that layer & then renamed the Walls, Beams layer.
You no longer have a layer named Walls, Beams. Chief X5 doesn't have a layer by that name anymore.
It's Ceiling Break Lines.Thanks, Jim
