Looking through the code a possibility is that a prior auto save has not completed or we are somehow getting another auto save before the first one completes.

That would be interesting indeed. Chief throws an error because the file is still locked by Chief's prior auto save operation.

Seems like you could test this hypothesis fairly easily in-house.
