Full shed dormer cape

Ok for all of those up in the Northeast how do I make a full shed dormer cape ???? One that has a 12 pitch on the front and a 5 pitch on the back and runs the full lenth of the house. I've looked at old threads and videos can't find one. I have been experimenting too with no luck.......
